APPLICANT REQUESTS THAT:

Her DD Form 214, Armed Forces of the United States Report of 
Transfer or Discharge, be corrected to include the Presidential 
Unit Citation (PUC), Air Force Outstanding Unit Award (AFOUA) 
and Basic Missile Occupational Badge.

A replacement DD Form 256AF, Honorable Discharge Certificate, be 
issued.  (Administratively Corrected)

APPLICANT CONTENDS THAT:

She served at Grand Forks Air Force Base as part of the 804th 
Supply Squadron, 804th Combat Support Group and her unit was 
awarded the PUC, AFOUA and Missile Badge.  These decorations and 
occupational badge was omitted from her records when she 
separated.

In support of her request, the applicant provided a copy of the 
“Wikipedia” page for Grand Forks Air Force Base and a copy of 
her DD Form 214.

The applicant’s complete submission, with attachments, is at 
Exhibit A.

STATEMENT OF FACTS:

The applicant initially entered the Regular Air Force on 12 Mar 
71.

The applicant was assigned to the 804th Supply Squadron, Grand 
Forks Air Force Base.

On 25 Aug 71, the applicant was furnished an Honorable 
discharge, and was credited with 5 months, and 13 days of active 
service.   

The applicant’s DD Form 214 reflects her occupation as an 
Inventory Management Specialist and lists the following 
Decorations, Medals, Badges, Commendations, Citations and 
Campaign Ribbons awarded or Authorized:  

	- National Defense Service Medal

The remaining relevant facts pertaining to this application are 
contained in the memorandum prepared by the Air Force offices of 
primary responsibility (OPR), which are attached at Exhibits C 
and D.    

AIR FORCE EVALUATION:

AFPC/DPSID recommends denial indicating there is no evidence of 
an error or an injustice.  

The PUC is awarded in the name of the President of the United 
States to units of the Armed Forces of the United States for 
extraordinary heroism in action against an armed enemy on or 
after 7 December 1941.  The unit must have displayed such 
gallantry, determination, and esprit de corps in accomplishing 
its mission under extremely difficult and hazardous conditions 
to have set it apart and above other units participating in the 
same campaign. The degree of heroism required is the same as 
that which would be required for award of the Distinguished 
Service Cross to an individual.
The AFOUA is awarded by the Secretary of the Air Force to units 
that have distinguished themselves by exceptionally meritorious 
service or outstanding achievement that clearly sets the unit 
above and apart from similar units.  All assigned or attached 
people who served with a unit during a period for which a unit 
award was awarded are authorized the appropriate ribbon if they 
directly contributed to the mission and accomplishment of the 
unit.

The applicant was assigned to the XXXXth Supply Squadron, XXXXXth 
Combat Support Group, Grand Forks Air Force Base, North Dakota, 
from 10 Jul 71 to 24 Aug 71.  While the applicant claims that 
her Group was reassigned under the XXXXXXXXst Strategic Missile Wing; 
they were unable to locate any official documentation to verify 
this restructure.

The applicant provided an Internet document showing the XXXst 
Strategic Missile Wing with the PUC and AFOUA.  This appears to 
be historical, as the unit was not awarded either of these 
awards during her assignment to Grand Forks Air Force Base.

AFPC/DPSIM recommends denial indicating there is no evidence of 
an error or an injustice.  

After consulting the functional advisor for the basic missile 
occupation badge it is determined the applicant is not 
authorized to wear this badge as she served in Supply and not 
Missiles.  The applicant failed to provide any certificate or 
evaluation documentation to support the authorization of the 
missile occupational badge.  The applicant’s Air Force Specialty 
was in Supply not Missiles.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

Copies of the Air Force evaluations were forwarded to the 
applicant on 1 May 15 for review and comment within 30 days 
(Exhibit E).  As of this date, no response has been received by 
this office.

THE BOARD CONCLUDES THAT:

1.  The applicant has exhausted all remedies provided by 
existing law or regulations.

2.  The application was not timely filed; however, it is in the 
interest of justice to excuse the failure to timely file.

3.  Insufficient relevant evidence has been presented to 
demonstrate the existence of an error or injustice.  We took 
notice of the applicant’s complete submission in judging the 
merits of the case; however, we agree with the opinions and 
recommendations of the Air Force offices of primary 
responsibility (OPR) and adopt their rationale as the basis for 
our conclusion that relief beyond that already granted 
administratively is not warranted.  Therefore, in the absence of 
evidence to the contrary, we find no basis to recommend granting 
the requested relief.

THE BOARD DETERMINES THAT:

The applicant be notified the evidence presented did not 
demonstrate the existence of material error or injustice; the 
application was denied without a personal appearance; and the 
application will only be reconsidered upon the submission of 
newly discovered relevant evidence not considered with this 
application.
